Provider Partnership Agreement
Effective Date: January 1, 2025 · Last Updated: May 2025
This Provider Partnership Agreement governs the relationship between LegacyBridge Care Advisors ("LegacyBridge," "we," "us") and care providers ("Provider," "you") who list their services on our marketplace platform. By creating a listing, you agree to these terms.
1. Platform Role
LegacyBridge operates as a marketplace and referral platform. We facilitate introductions between families seeking senior care and qualified care providers. LegacyBridge is not an employer, staffing agency, or healthcare provider. Leads provided are referrals only — LegacyBridge does not guarantee placement or conversion.
2. Listing Rules
Providers agree to maintain listings that are:
- Accurate and up-to-date regarding services, location, pricing, and capacity
- Free from misleading, exaggerated, or false claims
- Compliant with all applicable advertising and marketing laws
- Reflective of services the provider is currently licensed and able to offer
LegacyBridge reserves the right to review, edit, or remove any listing that does not meet these standards.
3. Lead Handling Expectations
When a family inquiry is delivered to your facility:
- Respond in a timely, professional, and compassionate manner
- Treat all lead information as confidential
- Do not share, sell, or transfer lead contact information to third parties
- Only contact the family regarding their specific care inquiry
Lead Ownership: Leads delivered through LegacyBridge remain the confidential property of the inquiring family. Providers receive leads as introductions only and may not redistribute them.
4. Free Trial and Subscription Terms
New providers receive a 30-day free trial that includes:
- Full listing visibility on the marketplace
- Access to incoming family inquiries relevant to your location and care type
- Dashboard access for managing your listing
After the trial period, continued access requires enrollment in an active subscription plan. Specific pricing and plan details will be provided separately. LegacyBridge reserves the right to adjust pricing with reasonable notice.
5. Legal Compliance
Providers represent and warrant that they:
- Hold all required federal, state, and local licenses and certifications to operate their care facility or agency
- Comply with all applicable health, safety, and employment regulations
- Maintain any required insurance coverage
- Will promptly notify LegacyBridge of any material changes to their licensing or operational status
6. Truthful Advertising
Providers are solely responsible for the accuracy of all information in their listings, including descriptions, pricing, services, amenities, and certifications. LegacyBridge is not liable for inaccuracies in provider-submitted content.
7. Termination
Either party may terminate this agreement at any time with written notice. LegacyBridge reserves the right to immediately suspend or remove a provider listing if:
- The provider violates these terms
- Complaints from families indicate misconduct
- The provider loses required licenses or certifications
- The listing contains false or misleading information
8. Indemnification
Providers agree to indemnify and hold harmless LegacyBridge, its officers, employees, and agents from any claims, damages, or liabilities arising from the provider's services, listings, conduct with families, or failure to comply with applicable regulations.
